Before beginning any task, identify hazards that could cause harm. Consider both how likely the harm is and how serious the consequence could be, then select appropriate controls in line with workplace procedures. After controls are in place, consider whether any residual risk remains. If the remaining risk is not acceptable, stop the task and speak with your supervisor before continuing. When recording a hazard or incident, separate observed facts from assumptions. Include the task, location, people or equipment involved, controls used, remaining risk and any further action required.
